Nav-CARE is a free volunteer navigation program that supports people with declining health to live well and independently at home for as long as possible. Think of this program as your "helpful neighbour".

Like a helpful neighbour or friend, our specially-trained volunteer navigators visit you in your home on a regular basis with a focus on enhancing your quality of life.

Assisting individuals in achieving their engagement goals by guiding them to identify and overcome barriers.

How It Works

Here's what typically happens after you contact us:

  1. Our team will refer you to the Nav-CARE Coordinator, who will guide you through the process.
  2. You will complete an intake form and discuss your service goals, likes, and dislikes.
  3. The Nav-CARE Coordinator will visit your home to further understand your needs.
  4. Goals of service, along with your likes and dislikes, will be reviewed to match you with a suitable Volunteer.
  5. Meet with your matched Volunteer and the Nav-CARE Coordinator for an introduction and to set expectations.
  6. Arrange recurring visits with your Volunteer at times that work for both of your schedules.
